Welcome We care for the whole person
and their family
The Holiday Season is an especially emotional time for everyone, but especially difficult for those who have suffered a recent loss.  The Light Up A Life memorial tree became a reality to honor and remember those who have departed.  It acts as a symbol of remembrance and compassion.  It is also a dynamic way to say, thank you Hospice, for caring.  This year our Light Up A Life trees are located at Arsenal Green in Malone, and next to the Food Court in Champlain Centres, Plattsburgh.

What is Hospice?
Hospice is about caring for those who are facing the challenges at the end-of-life. Those challenges may be physical, emotional, psychosocial, and spiritual. Hospice cares for the whole person and their loved ones, supporting them with skilled nursing, family services, home health aides, spiritual support, volunteer services and bereavement and grief support.

Who is Hospice?
Hospice of the North Country is a dedicated team of professional staff and trained volunteers who work with the patient's own physician, and other community resources, to provide patient-focused, family-centered, end-of-life care. This care is given without regard to disease, age, race, gender, orientation, color, creed or ability to pay.

Where is Hospice?
Hospice of the North Country cares for those with a life-limiting illness in the comfort of their own home or chosen setting (hospital, nursing home, assisted living, adult care home) and in the company of family and friends.
